As the owner of the above pet(s), I agree that my pet(s) is friendly and non-aggressive. I am responsible for medical expenses and/or damages to property, persons or other pets caused by aggressive behavior. I understand that Audubon Veterinary Hospital can terminate this contract in the event my pet(s) becomes a threat to the safety or health of Audubon Veterinary Hospital representatives. Audubon Veterinary Hospital will attempt to contact me in the event that an issue arises. I agree it is my responsibility to find other arrangements for my pet(s) immediately. Audubon Veterinary Hospital cannot guarantee against accidents and are not liable for loss or damages caused by or to our guests at this facility. Some pets may experience additional stress in the boarding environment. Audubon Veterinary Hospital is devoted to making the boarding experience as stress-free and as close to your home as possible.

Vaccines and External Parasites


In order to protect the health of your pet, this facility requires documentation of proof that your dog(s) has current Rabies, Bordetella, DA2PP and Lepto vaccinations. Cats are required to have current Rabies and FVRCP vaccinations. Pet(s) that are too young to receive the required vaccinations are not protected and the Owner accepts any risk of infection. As the owner of the above listed pets, I understand that my pet(s) will be examined for the presence of external parasites (fleas, ticks, etc.) before entering the boarding facility and treated accordingly. By signing below I agree to pay all related expenses associated with these treatments.

Diet


Audubon Veterinary Hospital provides a high quality diet for our borders. If you choose you may provide another commercial or prescription diet. If your pet has an upset stomach while boarding we provide Gastrointestinal Biome for a small charge per cup.

Medications


If your pet is receiving medication during your boarding stay, it must be provided in the original veterinary labeled container with instructions. As the owner of the above stated pet(s), I agree to pay any Fees associated with medications that need to be refilled during the time my pet(s) is boarded at this facility.

Statement of Kennel Policy


  • Boarding will be charged on a per night basis
  • Pets must be checked out or in by 4:45 p.m. Monday-Friday. Audubon Veterinary Hospital recommends you drop off your pet early enough to get medications if needed, walked, and settled in. Boarding charges start the day of drop off regardless of check-in time.
  • Discharges after hours or non-business hours are not allowed. If you are late you must pick up the next business day.
  • Pet(s) that are not picked up on the day of discharge, will be charged a late fee unless prior approval has been given. Please call immediately if you will not make your pick up appointment.
  • If we are not notified within 10 days after checkout day, the pet will be considered abandoned and will be handled in accordance with state law.

If personal items are left with your pet, you do so at your own risk and are solely responsible for loss or damage.

Payment


I understand Audubon Veterinary Hospital charges for boarding space per night. I agree to pay the rate of boarding in effect on the day my pet(s) is checked into the boarding facility. Payment is due upon checkout unless prior arrangements have been made and approved by Dr. Morgan. If any charges are not paid when due, interest will accrue at 1.5% per month, or the maximum rate allowed by law. All collection expenses, including attorney fees, will be paid by the owner.

This facility cannot guarantee the health of any animal, but pledges to provide appropriate care to all boarders. I agree to not hold this facility responsible for conditions that are unavoidable or any harm to my pet(s) from acts of nature beyond their control, such as hurricanes, tropical storms, tornadoes etc.


This document serves as authorization to Audubon Veterinary Hospital to provide veterinary care for your pet(s) in the event they are unable to reach you. This contract is legal and binding!

Staffing at Audubon Animal Hospital varies by season and days. If at any time a staff member is not present on the premises, the hospital is locked and monitored by an off-site alarm company. The safety and well being of your pet(s) is of utmost importance to us!
Any controversy or claim arising out of or relating to this contract, or the breach thereof, or as the result of any claim or controversy involving the alleged negligence by any party to this contract, shall be settled by arbitration in accordance with the rules of the American Arbitration Association, and judgment upon the award rendered by an arbitrator may be entered in any jurisdiction thereof.
I hereby unconditionally release, hold harmless and indemnify Audubon Veterinary Hospital, its employees, volunteers and representatives of and from all claims, liabilities and losses arising out of or in connection with the Audubon Veterinary Hospital use of work. The release and indemnification shall be binding upon me, and my heirs, executors, administrators and assigns.
